Step 1: Camp Payment

I understand that soccer is a physical sport involving running, kicking, and contact, which carries inherent risks of injury. I voluntarily assume all risks associated with my child's participation in the soccer camp, including but not limited to sprains, fractures, heat-related illnesses, or contact with other players and equipment.

I hereby release, waive, and hold harmless Charles Pait, All in 1 Coach, the camp coaches, staff, and the facility owners/hosts from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature, occurring during the camp.

In the event of an injury or medical emergency, I grant permission to the camp staff to administer basic first aid and, if necessary, to secure professional medical treatment or transportation to a medical facility. I accept full financial responsibility for any medical expenses incurred.

I grant All in 1 Coach permission to use photographs or video footage taken of my child during camp activities for promotional, website, or social media marketing purposes. (If you wish to opt-out of the photo release, please note it in the registration form comments).
